A third canal is infrequently seen in the mesial root of mandibular first molars, which is referred to as “middle mesial canal”. A survey of the endodontic literature reveals that the prevalence of the middle mesial canal is 115%. In this article, we report the successful endodontic management of a mandibular first molar presenting with five canals (three mesial canals & two distal canals). This paper also comprehensively reviews the prevalence of the middle mesial canal in mandibular molars amongst all ethnicities. It highlights the various diagnostic armamentarium which aid the clinician to detect additional canals to accomplish predictable clinical results.
