This clinical article presents endodontic management of a mandibular first molar with 3 roots (radix entomolaris) and a Vertucci type IV canal configuration in radix entomolaris. The presence of type IV canal configuration in entomolaris has never been reported in literature. Root canal treatment may be performed in root canal systems that do not comply with the normal anatomic features described in standard textbooks. Hence it is considered important to be familiar with variation in tooth canal anatomy and must be taken into account in clinical and radiographic evaluation during endodontic treatment.
