Ameloblastic carcinoma is a rare malignant lesion with characteristic histologic features and behavior that dictates more aggressive surgical approach than that of a simple ameloblastoma. However, reliable evidence of its biologic activity is currently unavailable due to the scarcity of well-documented cases. Majority of cases occurs in the mandible and presumed to arise de novo with few cases of malignant transformation of ameloblastoma. Clinically, it presents as a swelling with pain, and parasthesia of lower lip. Ameloblastic carcinoma is more aggressive than conventional ameloblastomas with extensive local destruction, perforation of cortical plate extending into soft tissue, tooth mobility and metastasis, usually to cervical lymph nodes. Histologically, the tumor cells resemble cells seen in ameloblastoma but show cytologic atypia, cellular pleomorphism, mitoses and, vascular and perineural invasion. These differentiating features of ameloblastic carcinoma must be known and recognized by the dental practitioner. We present a case of ameloblastic carcinoma of mandible associated with supernumerary teeth in a 40 year male patient.
