Periodontal bone grafting in the past has been controversial and unpredictable. One of the biggest challenges remaining in dentistry is to predictably regenerate the alveolar bone destroyed by periodontitis. Among the available bone grafts, autogenous bone is the material of choice followed by allografts and alloplasts. The limited predictability of bone grafting reported in earlier studies may soon be overcome by several innovative approaches that show encouraging results in early reports. Strong proponents of bone grafting argue that the majority of healing studies show better success using graft materials than open flap debridement in managing severe osseous defects.
