Maintenance of Records… … How Vital ?

Medical record is the ‘measure’ through which the issues (legal, medical, research) are evaluated at a later date, may be after years in occasional cases. The old dictum, “ if you didn't write it down, it didn't happen” is liable to be envoked in a legal sense. and conversely whatever is written is likely to be microscopically viewed through eagle-eyed attorney's. Instances may be legion where cases have been lost because of falling short of standards of maintaining medical records accpetable by honourabe courts, even though malpracticce/negligence in treatment/Or diagnosiss could not be proved for both coexisting as discussed in following article.
