Morphometric analysis of orbital aperture and frontal sinus using cone beam computed tomography as an aid in gender identification in forensic dentistry-a retrospective study

Abstract

Personal and gender identification of human skeletal remains plays a crucial role in case of mass disasters. Orbital aperture is unique and have higher resistance towards fracture. The radiographic pattern of frontal sinuses is unique to every individual and remains stable throughout the life. Frontal sinus and orbital aperture can be used reliably in human identification provided both antemortem and post mortem radiographs of the victims are available.

A retrospective study involving 100 CBCT images from 18-65years of age were retrieved from the department data base. Each image was analyzed using Planmeca Romexis software with the image thickness of 1.2 mm. The maximum height of the orbit, orbital width, orbital index, interorbital distance was measured in coronal section. The bizygomatic width was measured in axial section. Volumetric measurements were made using the “ellipse” feature of Romexis in coronal, sagittal and axial reconstructions. Frontal sinus length (width), frontal sinus anteroposterior length (depth) measured in axial section, frontal sinus height was measured in coronal sections. All measurements were expressed in millimeters except volume of the orbit was expressed in cubic millimeters

Multivariate logistic regression test was done to find the significant parameters in gender discrimination. P value of 0.002 for orbital height on the right side, 0.002 for orbital width on right side, 0.003 for orbital index on right side, 0.042 for orbital volume on left side, 0.023 for inter orbital distance, 0.011 for frontal sinus length on left side, 0.002 for frontal sinus height on left side was obtained.

As dental professionals, we can continue to play a key role by maintaining quality records and thus could be a part of the investigating team in the field of forensics. This method should be considered as one of the reliable methods for gender assessment because of its accuracy in living individuals of unknown personal data for human biology studies and, also in the forensic context.
