Griseofulvin induced Stevens Johnson syndrome (SJS). A rare case report from district headquarters Hospital, Chittoor, AP.

Drug induced adverse drug reactions and events are quite common nowadays. Among those stevens-johnson syndrome (SJS) is one of the rare Severe Cutaneous Adverse Drug Reaction (SCAR) which is characterized by typical peeling of skin, mucous membrane and epidermal detachment. The incidence of Griseofulvin induced Stevens-Johnson syndrome is less than 1%. Here the patient initially went to local doctor for complaints suggesting fungal dermatitis like diffuse itching following which she was started on Tab Griseofulvin 500mg BD for 5 days. The next day she came to district govt. hospital with complaints of typical peeled of lesions and pain over face, arms, trunk and peritoneal area for which topical steroid and oral corticosteroid was advised for 2 weeks with tapered doses in between after stopping Tab Griseofulvin was advised by the treating doctor. Later Histopathological examination of the skin sample was performed. which revealed the findings of SJS. The causal relationship of tab griseofulvin with SJS as per ALDEN Algorithm in one of the standard Euro SCAR study was found to be certain. The causality assessment score for this event SJS was found to be 7 as per naranjo’s ADR probability scale, henceforth the causal relationship with ADR is found to be certain . Treating physicians especially dermatologists must be more aware and vigilant before prescribing and advising Griseofulvin for any fungal diseases by doing regular follow-up of patients if advised and assessment of early diagnosis of these similar reactions could benefit the society by preventing the occurrences of SJS in future.
