Renal Safety and Antibacterial Efficacy of Biapenem in ICU patients with severe Infections: A Prospective Observational Study

Abstract

Biapenem, a newly developed parenteral antibiotic from the carbapenem class, exhibits broad-spectrum antibacterial activity against both aerobic and anaerobic bacteria. It is particularly notable for its stability against human renal dihydropeptidase-1 (DHP-1), making it a promising option for critically ill patients, especially those with renal impairment. This study aimed to evaluate the effects of Biapenem on renal function and its efficacy in treating infections in intensive care unit (ICU) patients, as well as to assess its safety profile. A prospective observational study was conducted over six months at the Critical Care Department of Care Hospitals, Hyderabad, involving 15 patients aged 18-80 with confirmed bacterial infections. Baseline measurements of serum creatinine, blood urea, and white blood cell (WBC) counts were recorded before Biapenem therapy, with follow-up assessments performed on day 2 after treatment initiation. Significant improvements in renal function were observed, with serum creatinine decreasing from 4.05mg/dL to 2.64mg/dL and blood urea dropping from 107mg/dL to 73mg/dL. WBC count decreased from 12,880 cells to 3,623 cells, indicating effective infection control. Platelet counts remained stable throughout the study.Biapenem demonstrated minimal nephrotoxicity and significant therapeutic efficacy in critically ill ICU patients, including those with renal dysfunction. These findings support its potential use for managing severe infections in this population. Larger studies are warranted to confirm long-term safety and effectiveness.
