The Indian system has been greatly influenced by the changes in political and economic thinking in India as well as the world. These changes get reflected in both the changes in approach to health care delivery and the structure of the delivery system. The approach to health care delivery in India was top-down, though great emphasis was placed on Universal access and equity. The Alma-Ata Declaration in 1978 shifted focus on democratization and its corollary, the primary health care approach. This ushered in changes at the structural level as well. In the above context, the present paper deals with the changes in approach to health care provision in India since its independence and to unearth the processes and events that have led to changes in the provision and structure of health care delivery system.
