A 26 year old Prim gravidae with 20 weeks of gestation was presented with altered sensorium, vomiting, Jaundice, fever, Abdominal Pain and constipation. A clinical Diagnosis was made as Acute Fatty liver during pregnancy with grade IV encephalopathy with no fetal movement. MTP was done. Liver Transplant was done for Acute Liver Failure. Postoperatively patient was stabilized and became conscious, eventually deteriorated with sepsis and thrombotic Thrombocytopenic Purpura. Despite the intensive care management in transplant unit, patient expired.
