Bartter syndrome is rare, salt-wasting tubulopathy with impaired reabsorption in the ascending limb of the loop of Henle, which results in hypokalaemia, hypochloraemia, and hypercalciuria. We report a rare case, a 5 yrs and 6 months old male child admitted in the Government hospital with recurrent vomiting for 6months, 4 episodes per day, Intermittent high grade fever and abdominal pain for 3 days. The child was diagnosed as Bartter syndrome, based on serum electrolytes assessment, 24 hours urine collection (calcium, phosphorus and creatinine) and arterial blood gas analysis. The child was initiated on Inj.Potassium chloride (KCL) 10mg in each 500ml NS @ 44ml/hour on flow.
