Electrical accidents in Power Sector i.e. Power Generation, Transmission, and Distribution Systems continue to be a major concern due to the involvement of high voltages, complex equipment, and human interaction. Despite the availability of safety rules, standards, and protective devices, accidents still occur, often resulting in serious injuries or fatalities. The accidents are due to either human error or system defective or environment. The types of accidents and root causes investigations are required. This paper presents fifteen real-life electrical accident case studies from substations, switchyards, transmission lines, and storage areas. Each case study focuses on the incident, root cause, and safety learning. The objective of this compilation is to analyze common safety failures such as unsafe acts, improper isolation, lack of supervision, and equipment defects, and to highlight preventive measures. Studying these cases helps engineers, supervisors, and workers understand practical hazards and reinforces the importance of strict adherence to electrical safety practices.
