The soft tissue covering the partially erupted tooth named as operculum and when this tissue become inflamed called as operculitis. Inflammation of partially erupted wisdom tooth, beneath the operculum commonly referred as pericoronitis. Acute pericoronitis presents with unbearable severe pain, fever and most of the time extraction of wisdom tooth is the only treatment for the patient. The incidences of pericoronitis are increasing year by year. The pathogenic bacterial strains causing pericoronitis are mainly anaerobic and belong to various species. The present case of pericoronitis was found in 34-year-old female Indian and major bacterial pathogen found was Streptococcus mutans. This pathogen was found susceptible against broad spectrum antibiotics and eradicated by empirical antibiotic treatment.
