Death due to mass envenomation by honey bee stinging

Abstract

India has a diverse range of venomous creatures capable of causing lethal injuries. Though, utmost attention has been paid on deaths caused by snake bites but arthropods such as honey bees (Apis melliferaj and European wasps (Vespula germanica), can also inflict dangerous stings. The incidences of honey bee stinging are common all over the world and severity of reaction to sting can vary from one person to another and even may vary in same individual at different occurrences. Most of the cases of honey bee stinging are usually uncomplicated because the signs and symptoms subside in a few hours after a brief episode of pain, wheal and flare reaction with local edema. Though, death due to honey bee stinging is rare but the fatal reaction can occur in cases of mass envenomation where a person is stung many times by a swarm of honey bees. Most of the victims of mass envenomation die either due to anaphylactic reaction within 24 hours or due to renal failure several days after the incidence. Detection of the cause of death in such cases is a difficult task for the autopsy surgeon especially in the absence of stinging and detailed history. One such case of mass envenomation was brought to us for autopsy examination in which we could be able to conclude the cause of death as ‘anaphylactic reaction due to multiple honey bees stinging’. The implications and proposed mechanism of anaphylactic reaction in this case with medicolegal aspects of honey bee stinging are discussed in detail as it is rare of its kind.
