A prospective analysis on the incidence and patterns of electrocardiographic abnormalities in 25 patients of acute aluminium phosphide poisoning, is presented ST and T changes were the commonest abnormalities. Various other abnormalities ranging from sinus arrythmias to bundle branch blocks were observed. The mortality rate in those with ECG changes was 24%. A unique observation in this study was the high mortality rates (50%) observed in patients presenting with ECG changes along with moderate to severe elevations in cardiac troponin T. Metabolic acidosis was the predominant acidbase disorder observed as one would expect in this poisoning.
