Sudden unexpected deaths due to cardiovascular causes are common. Due to the emergence of newer entities of heart diseases, the autopsy diagnosis becomes difficult. Right ventricular fatty disease, a genetically predisposed condition is one amongst them. Due to the infiltration of fat the normal myocardial architecture is disrupted. Death is almost always due to arrhythmias. Very often, due to the absence of morbid anatomical findings, the autopsy will be obscure. Histopathological examination of the myocardium can reveal the underlying pathology. A case of sudden death due to right ventricular fatty disease is reported.
