Diabetic ketoacidosis – An atypical presentation with olanzapine toxicity

A young healthy male was brought in emergency department of Sh. Guru Ram Dass Hospital in coma and signs and symptoms of extrapyramidal tract lesion, polyuria, dehydration, hypotension, Tachaponea, hyperpyrexia and excessive frothing from mouth. The past history revealed that patient was a case of schizophrenia and was on olanzapine. The investigations revealed that patient had high blood sugar levels; urine for ketones positive and other tests were in normal range. The detailed history from patient and relatives revealed that patient had consumed excessive dose of olanzapine. Patient was treated for olanzapine poisoning and other supportive measures. The condition of patient improved over few days and was discharged in satisfactory condition.
